Health premiums rose 31% in two years—but agents serving lower-income households lost 81% of their health sales
Health premiums grew, but health premium sold through micro-insurance agents—the channel designed to reach lower-income families—fell sharply. Most of the increase in people covered came through group policies.
Counting changed part-way: from 1 October 2024 IRDAI took premium on long-term policies out of reported premium, so 2024-25 is not counted quite like 2022-23. The council does not say by how much; the full note and source are in the evidence below.
Who gained health cover: people covered, year to March 2023 against year to March 2025
The counts behind this chart
| Who | Year to Mar 2023 | Year to Mar 2025 | Change |
|---|---|---|---|
| Group policies | 1,994 lakh | 2,751 lakh | +38.0% |
| Family floaters | 422 lakh | 496 lakh | +17.7% |
| Individual policies | 107 lakh | 105 lakh | −1.9% |
| Government schemes | 2,977 lakh | 2,454 lakh | −17.6% |
Who this touches
For businesses already serving lower-income households, this raises a distribution question. Could existing customer relationships help with insurance access and claims support?
The figures do not show whether families previously served by micro-insurance agents lost cover or moved to another channel. Establishing that comes before treating the decline as a business opportunity.
Health premium rose 31% over the two years to March 2025, but the people newly covered came mainly through group policies, while health cover sold by micro insurance agents, the sellers IRDAI created to reach low-income families, fell 81% over the same two years. Why micro agents sold less is not said anywhere, and the figures do not show whether the families they served lost cover or moved to another channel. Our inference, not IRDAI's: commission money moving to brokers' retail lines, where IRDAI prints the rate on retail health tripling, may help explain why the micro agents' health sales fell 81%.
Why we think so2 reasons
On what grounds?
- The first reason is IRDAI's own description of the channel that is shrinking. IRDAI says the micro insurance agent, introduced in 2005, was "aimed at attracting more intermediaries to this space and leverage upon the connect enjoyed by grassroots organizations like cooperatives and SHGs with a large section of the low-income segment." That channel's health sales fell from Rs 65 crore to Rs 12 crore in two years, shown at the top of the page. IRDAI's words say who the channel is for, not why its sales fell.
- The second reason is narrower, and it is about how the headline premium is counted. The General Insurance Council notes: "IRDAI has recently revised the formats for reporting and they have excluded premium from long term policies from reporting of premiums with effect from October 1, 2024." So the year to March 2025 is counted differently from the year before it, and the 31% two-year rise spans both ways of counting; the council does not say how much the change moved the total.
Why: where the selling money went
- Across all general insurance, brokers' commission grew almost five times as fast as the premium they sold. IRDAI's paper prints broker premium up 37% and broker commission up 173%, year to March 2023 to year to March 2025. These figures cover every line brokers sell, not health alone.
- Retail health is where the rate rose most. IRDAI prints commission on retail health sold through brokers rising from around 10% to 30% over those two years.
- Retail lines earn brokers more than they sell. IRDAI prints them at about 54% of broker premium but about 78% of broker commission in the year to March 2025.
- Some health deals pay far more. IRDAI prints payouts in "certain corporate and group arrangements" reaching as high as 93%.
- The market is narrow. IRDAI prints the top six insurers at about 65% to 68% of retail health, ten states at 80% to 82% of retail health policies, and Maharashtra alone at 20%.
What would prove us wrong3 trip-wires
What would we have to see to drop that read?
- If next year's handbook shows micro-agent health sales recovering, the fall over these two years was a passing dip and not a channel losing its place.
- If the drop in government-scheme coverage turns out to be families moving onto group cover, the people the micro agents served were covered another way, and the strain is smaller than it looks.
- If the change in how long-term premium is counted from October 2024 also moved the channel figures, part of the micro-agent fall is a counting effect and not lost sales.
What to watch3 things to watch
What happens next, and when would we know?
- Watch the next IRDAI handbook's micro-agent health and group-life lines: another year of sharp falls would show the channel shrinking rather than dipping.
- Watch one-person health cover, which barely moved in two years: a rise there would mean individuals, not only groups, are starting to buy for themselves.
- Watch how many gig workers the government actually enrols under PM Jan Arogya Yojana, and whether government-scheme coverage stops falling once they join.

Micro agents' other sales: group life fell, while direct online health sales rose
The government's gig-worker announcement, beside falling scheme coverage
The labour ministry's release of 2 Feb 2025 says gig workers are to get "healthcare security under PM Jan Arogya Yojana", calling it "a transformative step towards their social security and well-being." That is one scheme for one group of workers. Across all government sponsored schemes, premium fell from Rs 10,514 crore to Rs 9,459 crore in the year to March 2025, and people covered fell from 2,611 lakh to 2,454 lakh in that year.
Health insurance premium, all insurers year by year
What these terms mean
What health insurance is, and who it is for. Health insurance is cover that pays hospital bills. Anyone can hold it: a person can buy it for themselves alone, a family can buy one policy that covers all its members, an organisation can buy one policy for a whole group such as its staff, and the government buys it on behalf of people it has chosen to protect through its own schemes.
Where the figures come from. The insurance figures on this page are printed by IRDAI, the insurance regulator, in its annual handbook of Indian insurance, except three named where they appear: the note on how premium is counted, the hospital cash price, and the broker and commission figures in "Why: where the selling money went", which come from IRDAI's Distribution Reforms — a proposal the regulator publishes to invite comment before it decides; it binds nobody yet (Part 1, September 2026). Health figures leave out personal accident and travel cover. The handbook prints premium in Rs lakh and people in thousands; the page shows premium in Rs crore and people in lakh, where a lakh is a hundred thousand.
What gross premium is. Gross premium is the money customers paid insurers for cover in the year, before anything is passed on to other insurers. Health premium was Rs 117,505 crore in the year to March 2025, up 9.1% on the year before.
A change in how premium is counted. The General Insurance Council, the industry body of general insurers, notes that IRDAI changed its reporting formats and took premium on long-term policies out of reported premium from October 2024. So the year to March 2025 is not counted quite the same way as the year before it.
The kinds of cover. The handbook splits people covered four ways. Group cover is one policy for a whole group. A family floater is one policy whose sum insured is shared by a family. Individual cover other than a family floater is a policy for one person alone. Government sponsored schemes are cover the government pays for. Government schemes cover many people at a low premium, so premium per person is only worth comparing within one kind of cover.
What a micro insurance agent is. A micro insurance agent is a seller IRDAI introduced in 2005 to bring more intermediaries into insurance for low-income families, working through the reach that cooperatives and self-help groups already have with them.
What direct online sale is. Direct online sale is a customer buying from the insurer's own website or app with no seller in between.
Group life cover. Group life new business is life cover newly sold to whole groups in the year, counted both as premium and as the number of lives covered.
What a unit-linked plan is, and what commission is. A unit-linked plan is a life policy whose savings part is invested in markets, much like a fund. Commission is what an insurer pays the seller for the sale, and this line includes rewards as well. It is one line of what life insurers pay sellers, not the whole of it.
What hospital cash is. Hospital cash cover pays a fixed sum for each day a person spends in hospital, whatever the bill. The price shown is one insurer's own filed rate, not an industry average.
